SHOW PARTICIPATION FORM

I agree to a three-day VS1 Games Miami participation window:

Day 1 — Arrival / Free Day
Arrival in Miami. No scheduled commitments.

Day 2 — Media Day
Participation in VS1 Games production, which may include scripted arrivals, interviews, photography, fashion and styling, hero shots inside the HexaCage, promotional filming and other reasonable production activity.

Day 3 — Gameday
Participation in the VS1 Games short-form 1v1 exhibition competition.

VS1 Games combines scripted sports entertainment with unscripted football competition. Match results and sporting performance are not predetermined.

Players participate as independent contestants in the show, not as employees, team members or players registered to VS1 Games or VS1 Miami.

Games take place inside the VS1 HexaCage.

Each matchup is played as a best-of-three-round contest. Both players receive one possession per round, with up to three opportunities to score during that possession.

Games are short by design, averaging approximately 10 minutes.

The competition is single elimination:

Last 16 → Quarter-Final → Semi-Final → Final

I will therefore play a minimum of one (1) game and a maximum of four (4) games, depending entirely on progression.

Once eliminated, I have no obligation to participate in any further games.

I understand and agree that my participation in this VS1 Games Miami edition is voluntary and unpaid.

No appearance fee, participation fee, prize money, NIL fee or other financial compensation is owed for my participation or the media and NIL rights granted under this Form unless separately agreed in writing with VS1 Games.

VS1 Games will provide the agreed accommodation, local transportation and hospitality associated with my participation.

I am responsible for arranging and paying for my own travel to and from Miami.

Once in Miami, VS1 Games will provide or cover agreed Games-related:

• —hotel accommodation;
• —local ground transportation;
• —airport transfers where arranged;
• —Games-related Uber or equivalent transportation;
• —Media Day and Gameday meals;
• —official VS1 Games dinners; and
• —reasonable hospitality during the participation window.

Personal expenditure outside the official VS1 Games programme remains my responsibility.

I authorise VS1 Games to photograph, film and record my participation in VS1 Games Miami.

I grant VS1 Games the worldwide right to use my name, image, likeness, voice, interviews and VS1 Games performance in connection with the production, distribution, promotion, marketing, sponsorship, social media, broadcast, streaming and archival use of VS1 Games and VS1 Games Miami.

All footage, photography and media created or commissioned by VS1 Games remains the property of VS1 Games.

VS1 Games will not knowingly use my participation, likeness, career or personal story in a manner intended to materially damage, defame or unfairly harm my professional reputation.

I understand that VS1 Games match footage, production footage, the HexaCage and production set are protected VS1 Games content.

Without prior written approval from VS1 Games, I will not publish or distribute:

• —unreleased game footage;
• —copyrighted VS1 Games footage;
• —unreleased production or behind-the-scenes footage; or
• —personal photographs or videos prominently showing the VS1 HexaCage or production set.

VS1 Games will provide participating players with an approved personal VS1 Games Media Package, which may include professional photographs, Media Day content, game highlights, short-form clips and other player-specific media.

I am free to post the approved content provided to me, subject to any communicated release date or embargo.

I agree to reasonably accept official VS1 Games social-media collaboration posts directly related to my participation, provided they do not create an unauthorised personal endorsement of a third-party product or service.

I will bring my own suitable football boots.

VS1 Games will provide official playing shorts, socks and applicable Games wear.

Any existing personal apparel or sponsorship restrictions should be disclosed to VS1 Games in advance so they can reasonably be accommodated.

For Media Day, I may be professionally styled by VS1 Games. This may include fashion attire, clothing, footwear, accessories, vehicles, props and other creative elements used to bring the VS1 Games production to life.

I understand that participation in VS1 Games involves the ordinary physical risks associated with football, athletic activity, filming and competition.

VS1 Games is a short-form off-season exhibition between professional footballers who voluntarily elect to participate.

I am responsible for determining that I am physically fit and medically able to participate.

To the fullest extent permitted by applicable law, I voluntarily accept the ordinary risks associated with participation and agree that VS1 Games, VS1 Miami, their personnel and production partners will not be responsible for injuries or medical conditions arising from my voluntary participation, whether occurring during Media Day, Gameday, warm-ups, filming or other physical activities forming part of the production.

VS1 Games may provide or arrange reasonable on-site first-response medical support.

If further medical assessment or treatment is required, VS1 Games may assist me in identifying or accessing an appropriate local hospital, clinic or medical provider. I remain responsible for any external hospital, medical, rehabilitation, medication or related costs unless separately agreed in writing.

Nothing in this Form excludes liability that cannot legally be excluded.

By signing this Form, I confirm that:

• —I voluntarily wish to participate in VS1 Games Miami;
• —I understand that my participation under this Form is unpaid unless otherwise separately agreed in writing;
• —I am responsible for my own travel to and from Miami;
• —VS1 Games will provide the agreed accommodation and Games-related on-the-ground hospitality;
• —I understand the three-day participation commitment and competition format;
• —I understand that VS1 Games combines scripted entertainment with unscripted sporting competition;
• —I understand that VS1 Games is a sports-entertainment show and exhibition competition, not a professional football league;
• —I participate as an independent contestant and not as a member of VS1 Miami;
• —I understand and voluntarily accept the ordinary physical risks involved;
• —I grant the media and NIL rights described in this Form;
• —I will comply with the VS1 Games content and social-media requirements;
• —I will disclose relevant sponsorship or apparel restrictions; and
• —I will comply with reasonable VS1 Games production, competition, safety and conduct requirements.
